  3. Next click on the "Fields" option in the customization panel
  4. Click the "Add" button in the Fields section

  6. Enter "Status" in the Field title input field
  7. Type in the desired name in the field title input text field
  8. Click the drop down arrow button under the field type
  9. Select the desired field type from the dropdown menu options
  10. Type the name you want to use for the option
  11. Finally click the "Create field" button to save the new field
