Here is how to group your form fields on jotform
- First click on a field label in the form
- Then click on the "Properties" button next to the Phone Number field
- Click on the "ADVANCED" tab in the Phone Properties dialog
- Click on "Shrink" toggle to make the field smaller
- Next click the second form you want to group
- Click on the "Properties" button next to the Email field
- Click on the "ADVANCED" tab in the Email Properties dialog
- Click on the "Shrink" toggle switch again to group it with the first field
