Here is how to create a contact information block on a survey in mailchimp
- First click on the "Audience" item in the left sidebar menu
- Then click on the "Surveys" item in the sidebar
- Next, click on the "Create new survey" button
- Then, click on the "Survey name" input field and enter your survey title
- After that, click on the "Begin" button
- Next, click on the plus icon to add a new question
- Then click on the "Contact information" option to add a contact information question
- Next, click on the "What would you like to ask" input field and enter your question
- Then click on the "Contact information category" dropdown
- After that, click on "Continue to design" button
- Next, click on the "Review" button
- Finally, click on the "Continue" button
