First select the survey you want to add matrix question and click the three-dots (...)
Then click on "Edit survey" option from the dropdown menu
In "Design Survey" tab, click on the dropdown arrow next to "New question" button
Select "Matrix / Rating Scale" from the question type options
Enter the question text in the question field
Under the "Rows" section, input the items or statements you want respondents to rate
Under the "Columns" section, enter the answer choices for each statement
Then click "Save" to add it to your survey
You have successfully added a Matrix question to your survey
