Here is how to add open-ended questions in surveymonkey

On your SurveyMonkey dashboard, choose the survey you want to edit from your list
Then click on "DESIGN SURVEY" tab in the survey navigation menu
Scroll down and click “+ New Question” where you want to add a question
Enter the question you would like to add
Click and browse the list of question types in the right-hand panel
Select either "Single Textbox" for short answers or "Comment Box" for longer responses
Click "Save" to add it to your survey
